Only members of the Scandia Clubhouse who pay Clubhouse assessments are required to sign this waiver to receive access to the pool. If you do not pay Scandia Clubhouse assessments, please do not fill this out. Additionally, your assessments must remain paid in full to maintain access to the pool.

**If you already have a pool fob from the previous tenant or last year's pool season, your fob will be activated soon.
**If you do not have a fob in your possession, you will need to pick one up at the HOA office (4350 W 2360 N, located just outside the main pool gate) M-F between 9am-1pm. The office is located just outside the south pool gate. The pool is tentatively scheduled to open on Friday, May 22nd, pending inspection.
WAIVER
I hereby acknowledge that I am owner or renter of said property of the Holbrook Farms Homeowners Association in good standing. I understand the risks involved in using the Holbrook Farms provided facilities and I accept any and all responsibility for myself, my family and my guests including, but not limited to property damage loss, bodily injury, and death. In consideration of my participation and representative of family and guests, I agree to indemnify, defend and hold harmless the Holbrook Farms HOA its officers, directors, employees, volunteers, and assigns from and against all claims arising out of or resulting from said participation for myself and all those that I represent; family and guests. I also understand that myself, my family, and guests are to behave in a safe and responsible manner at all times while attending the Holbrook Farms Pool and in compliance with its pool rules. I also understand that if I, or they do not comply with the rules and regulations privileges may be revoked. Also, any renter/tenant(s) hereby agree to indemnify and hold harmless Holbrook Farms HOA against any all liability, claims, suits, losses, costs, and legal fees caused by the use of photographs for residential newsletters, posters, advertisements, flyers or emails. I attest the above information is correct and agree to follow all Holbrook Farms Association CC&R’s, Design Guidelines, and Rules.
I authorize my child(ren), named below whom I certify are at least fourteen (14) years or older to receive a pass to gain entrance to the Scandia Clubhouse Pool and its amenities without my attendance. I agree to hold harmless the Association from and against any and all claims, actions, damages, liability and expense arising out of or in connection with attendance and use of the pool and or other facilities by my child(ren).
POOL RULES
Lifeguards are not on duty. In the event of an emergency, call 911.
Bathers should not swim alone. Children 14 and under shall not use pool without responsible adult
supervision.
Elderly persons and those suffering from heart disease, diabetes, or high blood pressure should consult
a physician before using the pool.
Persons suffering from a communicable transmissible via water may not use the pool. Persons using
prescription medications should consult a physician before using the pool.
Individuals under the influence of alcohol or other impairing chemical substances should not use the
pool.
Pregnant women should not use the pool without consulting their physicians.
Running or engaging in unsafe activities or horseplay in or around the pool is prohibited.
Use of bikes, scooters, roller blades, and other wheeled toys are prohibited on the pool deck.
Diving in shallow water is prohibited.
A bather using the facility must take a cleaning shower before entering the pool enclosure. A bather
leaving the pool to use the toilet must take a second cleansing shower before returning to the pool
enclosure.
A person having any exposed sub-epidermal tissue, including open blisters, cuts, or other lesions may
not use a public pool. A person who has or has had diarrhea within the last two weeks caused by an
unknown source or from any communicable or fecal-borne disease may not enter any public pool.
Any child under three years old, any child not toilet trained, and anyone who lacks control of defecation
shall wear a water resistant swim diaper and waterproof swimwear. Swim diapers and waterproof
swimwear shall have waist and leg openings fitted such that they are in contact with the waist or leg
around the entire circumference.
